New Patients
Welcome to our practice. We are pleased that you have chosen our office to participate in your medical care. Please take the time to look around our website and familiarize yourself with Dr. Pavithran and the practice of nephrology.
What to expect at our office - your vitals will be taken and Dr. Pavithran will discuss the information that she has received from your referring physician and a plan of care will be created for you. Please bring the names of other doctors that you are seeing. After your visit, Dr. Pavithran will write a letter detailing your condition and send it to you and your reffering physcian.
At your first appointment we ask that you bring a current medication list. Please bring the details of the prescription etc., If you are have any other information around your health like blood pressure logs etc. please bring those as well.
Please make sure that your new patient paperwork is completed prior to arriving to our office or you can do this prior to the appointment, at the very least these need to be filled out before you leave our office on your first visit.
Existing Patients
Please check back here for links to documents that you can pre-fill before your visit and submit via Microsoft HealthVault systems or Google Health Profile sharing.
We will accept these profile and EHRs if you have already registered with these systems, please provide the information we need to access these very private records only when you are our office location. Thank you.
If you would like to fill out our paper work in advance to coming to our office location (like insurace and demographic information - we have set up a collaborative workspace for you to interact with our office staff electroncally.
The first step to enabling your access (we need your email address to send you an invitation for secure access via HTTPS) is to you the Contact Us Page and asking us that you would like to participate in this test program. Once we receive this request, you will get an invitation to use our workspace where you can download the latest new paitent forms, and print them out and bring them into our office at your scheduled appointment time.
We are cutting edge in our best practices around using technology and very green so we will scan these documents and put in a secure area where you can reach acess them in a secure and private area.
